XERF vs HIFU Treatment: Find the Best Skin Tightening Option for You

XERF and HIFU both tighten skin without needles or surgery, but they work differently. XERF heats the skin with radiofrequency energy across several depths at once. HIFU focuses ultrasound energy into small points at set depths, down to the layer a surgical facelift works on. Neither one is "better" for everyone. The right choice depends on your skin, your comfort and how much downtime you can accept.

Head-to-head research on the two is still limited. Most of what we know comes from studies of one device at a time. This guide sets out what each treatment does, where they differ and what to ask before you book.

What is XERF?

XERF is a non-invasive skin tightening device made by Cynosure Lutronic. It uses monopolar radiofrequency (RF) energy at two frequencies, 6.78 MHz and 2 MHz, delivered in a single shot. The manufacturer says the two frequencies heat tissue at more than one depth, including the fibrous bands under the skin that help hold it in place.

The handpiece cools the skin surface as it works. That cooling is the main reason an xerf treatment is often described as warm rather than painful.

XERF launched in 2024 and received US FDA clearance in 2025. Read the fine print, though. The clearance covers electrocoagulation, and the manufacturer explains that "skin tightening" refers to tightening that happens through soft tissue coagulation.

What is HIFU?

HIFU stands for high-intensity focused ultrasound. For the face and neck, the more precise term is micro-focused ultrasound (MFU), and Ultherapy is the best-known brand. The device sends ultrasound energy to a focal point beneath the skin, where it creates tiny points of heat while leaving the surface untouched.

Downtime and results

Both treatments are known for little downtime. You can usually go back to normal plans the same day.

After XERF

Some redness or warmth for a few hours. Swelling is less common but possible.

After HIFU

Redness, mild swelling or tenderness for a few days.

Neither XERF nor HIFU delivers an instant facelift, as both treatments stimulate collagen production and improve skin firmness gradually. XERF results typically reach their full effect after around three months and can last up to a year, while HIFU results usually develop fully within six weeks and last around six months. The duration may vary depending on your skin condition, age, and lifestyle.

Who chooses which?

XERF often suits people who:

  • Want to lift and tighten loose skin on the face, jawline or neck
  • Do not want to lose facial fat, for example because their face is already slim or starting to look hollow
  • Prefer a comfortable treatment without numbing
  • Want to be back at work or out to dinner the same day

HIFU often suits people who:

  • Want to slim chubby cheeks or reduce a double chin
  • Want a sharper, more defined lower face
  • Are looking for a more affordable way to start

The reason is how each one works. XERF heats the skin to tighten it without reducing facial volume, which matters if your face is already slim. HIFU can also reduce small pockets of fat, which helps with fuller cheeks and the area under the chin. HIFU is also the lower-cost option.

Neither replaces surgery for heavy sagging. If your skin hangs well below the jawline, a surgeon's opinion is worth getting first.

What to ask in your consultation

An experienced practitioner will examine your skin condition before recommending one of these devices. Dr Irene, an experienced HIFU in Bali, will be happy to answer any questions you may have, such as:

  1. Which device brand do you use, and is it a genuine, approved system?
  2. Who performs the treatment, and what training do they have?
  3. Which depths or settings do you plan to use for my skin?
  4. How many sessions do you expect I will need?
  5. Have you treated skin like mine, including darker skin tones or skin prone to pigmentation?

Tell the practitioner about pregnancy, a pacemaker or other implanted device, metal implants in the treatment area, recent fillers, or a history of hyperpigmentation. Some clinics also ask you to pause retinol for a few days beforehand.

How to decide

Think about what bothers you most when you look in the mirror.

  • Mostly a loss of firmness or crepey texture? XERF is a gentle place to start.
  • Mostly sagging along the jawline, chin or brows? HIFU's deeper focal points are designed for that.

Our Clinics Has Everything You Need

XERF and HIFU are both good options for tighter, firmer skin without surgery. XERF leans towards comfort and overall firmness. HIFU leans towards depth and targeted lifting. Frequently Asked Questions

Is XERF less painful than HIFU?

Usually, yes. XERF is often described as a warm sensation, helped by a cooling tip, and our clinic treat without numbing cream. HIFU tends to feel more intense, especially over bony areas. Pain levels vary with the person and the settings used.

Does XERF reach as deep as HIFU?

Yes. XERF is a new-generation monopolar RF treatment that targets three tissue depths: superficial, middle, and deep. It can reach deeper layers, including the SMAS, just like HIFU.

How long do XERF and HIFU results last?

XERF results typically reach their full effect after around three months and can last up to a year. HIFU results usually reach their full effect within six weeks and last around six months. We recommend maintenance sessions based on your age, skin condition, and lifestyle to help maintain your results.

Can I get an xerf treatment and HIFU together?

Some clinics combine them, using HIFU for deeper lifting and XERF for overall firmness. The order and timing should be planned by your practitioner. Do not book them back to back on your own.

Is there downtime after HIFU or an xerf treatment?

Very little. Expect possible redness after either treatment. HIFU may also leave mild swelling or tenderness for a few days. Most people return to normal activities the same day.
